HOW TO REGISTER FOR SCHOOL

Required documents for registration:

1. Application form (załącznik.pdf),

2. Parent/Guardian residence information (załącznik.pdf),

3. Status of foreigners (załącznik.pdf),

4. Previous school graduation certificate with the grades report (translated into Polish language),

5. Immunization/Vaccination card (translated into Polish language),

6. Parents’ and child’s passport  (if a child owns a passport).

You can print and fill in the necessary documents yourself, and then take them to school OR come to school and fill the documents in with the help of a school staff member.

PESEL – Personal Identity Number What is it? How to get it?

Each person living in Poland (Poles and foreigners) identifies himself/herself with own individual number, which is required everywhere: at school, at work, at the medical services. LUNCH AT SCHOOL – Purchasing procedures

1. If you order lunches for the first time:

a) Pay for a MAGNETIC LUNCH CARD, which records the information about purchased lunches. The payment should be made by bank transfer to the following account: 43 1030 1508 0000 0005 5080 5065. In the title of the transfer, write: name, surname and class of the child the lunches are for.

b) Pay for lunches for a given month (the subscription price for a given month is given here:

- in cash in room 13 (ground floor),

- or by bank transfer to the following account: 21 1030 1508 0000 0005 5080 5073. In the transfer title, write: name, surname and class of the child and the month for which the fee is charged (for example, Sam Smith, class 4a, September 2021).

c) Come to room 13 with a transfer confirmation and pick up a magnetic lunch card.

d) Thus, the child can eat lunch at the school canteen (he/she must have a magnetic lunch card with him every time).

2. You want to order lunches once again:
